SUBMISSION REQUIREMENTS	Min 5 years currently valued Loss Runs, with details on losses over $50,000 incurred Past 5 years annual Gross Receipts, annual Total Mileage, and average Power Unit count for each of the 5 years Complete Drivers List of all drivers, including full name, state of license, drivers license number, date of hire/seniority and a copy of recent MVR (Will accept copies of MVR&s from account&s Driver Files for Quoting). List of all company owned and owner-operator vehicles, including: year, make, model, weight, VIN (last 5 digits) and current market value & garage location for each. (Acord App OK). Completed Supplemental App Required. (See Above for Copy). If Long Haul, copies of past 4 Quarter&s IFTA - Schedule B Reports (International Fuel Tax Administration Schedule B). IF DOT filings, copies of all operating authorities and copy of most recent state or federal compliance review and current safety rating notice (both sides). Copy of fleet safety and maintenance program and key written company safety policies. Copies of all permanent lease and trip lease agreements, hold-harmless, interchange, interline, intermodal and sub-hauler agreements. If more than 25 power units, copies of account&s past two years Profit & Loss Statements and Balance Sheets. (Parent companies financials too, if applicable).
