SUBMISSION REQUIREMENTS	Signed Accord Application (125 and 130 Sections) Resume on owner(s) or key employee(s) Current and 3 years prior hard copy loss runs valued within 120 days of the proposed effective date Detailed description of all claims with an incurred amount over $25,000 Copy of current Experienced Mod Worksheet or Snapshot Detailed description of operations

CLASSES	Most all Major classes of business are accepted EXCEPT for the following: PEO’s, Staffing or Employee Leasing Firms Paper General Contractors, & Project Managers Boat Cruises and Water Taxis Hospitals, Hospice, Traveling Nurses, Assisted Living Facilities, Convalescent or Nursing Homes, Group Homes First Responders- Policy, Fire, and Paramedics Other classes of business may vary by state
PREMIUM	Premiums from $1500 and above
DEDUCTIBLE	Statutory deductibles as approved by the State
